To access the treatment please download, fill and sign the waiver of liability and consent form before placing any order. We will not process orders before receiving the signed document.

Send YOUR INFO

Please post the signed waiver in your FIP Warrior treatment chat.

• One waiver per cat covers the entire treatment period

• PDF or screenshot of waiver acceptable

PLACE YOUR ORDER

Once we receive your signed waiver and the requested information we will send you the code to access to the ordering page. If you are experiencing issues please contact your FIP Warrior admin.

NOTE: FOR RELAPSE COVERAGE, You must enrolled in Oasis case management software from the beginning of treatment and keep it up to date to be eligible. Please contact us if you are not signed into Oasis or prefer to use a google form.
